We are currently seeking a Full-time Registered Nurse Case Manager to work in our client's worksite Health & Wellness Center in Jersey City, NJ! This position is required to work Monday-Friday 8:30am-6pm as well as be part of an on-call rotation schedule (roughly one weeknight per week and every 5 th weekend).
Responsibilities:
• Assesses the coordination and access to needed medical, psychiatric, social, educational, and other services essential for meeting the health needs of the individual
• Coordinates assessment, treatment planning, and return to work processes (if applicable) with the employee, providers, and employer
• Assesses and evaluates cases and prepares and implements treatment and case management plans
• Counsels clients, assisting them to identify and resolve problems and make effective use of resources
• Maintains collaborative working relationships with other community providers to develop specialized services as required
• Perform duties telephonically
• Acts as liaison between client employees, providers, and agency or institution
• Provides consultation to staff on specific cases
• Prepares and maintains written case records, reports, and forms, performs case follow-up and closing, and performs other administrative tasks as required
• Other duties may be assigned

Requirements
• Associate's degree in Nursing required. Bachelor's degree in Nursing (BSN) preferred.
• Current license as a Registered Nurse in the state of New Jersey required. In addition, a current RN license in the state of New York highly preferred.
• Minimum three years experience as a Registered Nurse in an acute hospital setting highly preferred. Previous corporate nursing experience also highly preferred.
• Minimum three years case management experience preferred.
• Certification in Case Management (CCM) preferred.
• Current BLS for Health Care Providers required. ACLS preferred.
• Strong communication skills required.
• K nowledge of Internet software, Spreadsheet software and Word Processing software required.
