Summary
The Case Manager coordinates the overall interdisciplinary plan of care for patient, from admission to discharge.
Responsibilities
- Consistent with professional standards, creates a quality, supportable plan for care across the continuum, integrating patient/family preferences and values into the plan.
- Ensures patient is progressing towards desired outcomes by monitoring care through assessments, evaluations and/or patient records.
- Advocates for resources and removal of barriers to promote outcomes that are consistent with patient values and preferences across the continuum.
- Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.
- Performs other duties as assigned.
Requirements:
Education & Experience
- Two years of clinical experience in an acute care setting required.
- Graduate of an accredited school of professional nursing required.
- Previous experience in case management or utilization review preferred.
- Bachelor's degree in Nursing preferred.
Licensure, Certification, and Registration
- Current licensure as a Registered Nurse (RN) in the state of Florida required.
- Current Case Management certification preferred.
