Cabrini Care at Home, a Licensed Home Care Services Agency, offering a full range of personal care, homemaker and other home health services 24 hours a day, seven days a week. The Long Term Home Health Care Program provides all of the skilled nursing care you would find at a nursing home, right in the comfort and privacy of your own home. Certified Home Health Aides needed for "Cluster Care Hours" Duties May Include: Provide routine, personal healthcare, such as bathing, dressing, or grooming, to elderly, convalescent, or disabled persons in the home of patients or in a residential care facility. Administer prescribed oral medications under written direction of physician or as directed by home care nurse and aide. Change dressings. Check patients' pulse, temperature and respiration. Direct patients in simple prescribed exercises and in the use of braces or artificial limbs. Maintain records of patient care, condition, progress, and problems in order to report and discuss observations with a supervisor or case manager. Massage patients and apply preparations and treatments, such as liniment, alcohol rubs, and heat-lamp stimulation. Provide patients with help moving in and out of beds, baths, wheelchairs or automobiles, and with dressing and grooming.
