Clinical Pharmacist - the Heart Hospital Baylor Denton - Full-Time - Weeken

The Clinical Pharmacist provides pharmaceutical care to patients. Compounds and dispenses medications and other pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents. Consults with Nursing, Medical Staff and patients regarding medications, orders, drug and pharmaceutical detail, patient reactions, errors and complaints and/or the patients' caregivers. May have responsibility for the assessment, treatment and/or care of neonates, pediatrics, adolescents, adults and geriatrics. (Note: Relevant Licenses would be as follows: Registered Pharmacist and Pharmacist Preceptor.) I. Fills Medications. * Compounds and dispenses medications based on organizational and regulatory guidelines. * Selects ingredients for medications and determines pharmaceutical, chemical, physical or physiological incompatibilities. * Prepares and maintains all necessary records on medications, patient charges and inventories. * Controls costs by using drug formularies and clinical initiatives where appropriate. * Supervises and directs the duties of pharmacy technicians in processing and distribution of medications. * Consults with Patients and/or Patient Caregivers. * Acts as a consult/educator to Nursing and Medical Staff concerning pharmaceutical care of patients. * Consults/educates patients concerning possible medications including alternative sources of drug therapy. * Ensures that patient care meets standards of safety, effectiveness, patient rights and customer service. * Assists medical staff in optimizing drug therapy. * Prescribes medications per approved protocols. * Evaluates special patient needs and provides appropriate pharmacy care such as renal dosing, hepatic deficiency, etc. * Monitors patient response to drug therapy. III. Assess physicians' medication orders with respect to the overall patient therapy to evaluate: * Untreated indications. * Improper drug selection. * Sub-therapeutic dosage. * Failure to receive medications. * Over-dosage. * Potential adverse drug reactions. * Potential drug interactions. * Medication use without indication.
