HIM Clerk III-PRN
GENERAL SUMMARY:
Performs the following principal duties as assigned:
Assembles and reviews inpatient, outpatient and emergency medical records to ensure completeness of documentation, or processes requests for patient information in response to proper written authorization
Conducts self and communicates with staff, physicians, other departments, and the public, in such a way that promotes positive morale, professional, and a knowledgeable image of the department.
Under general supervision:
1. Assembles and reviews inpatient, outpatient and emergency medical records to ensure completeness of documentation in accordance with department policies and procedures. Researches missing documentation as needed. Communicates documentation deficiencies to physicians in writing and/or verbally as appropriate. Acts as information resource for other department staff and physicians regarding record completion requirements.
2. Processed requests for patient information in response to written authorizations in accordance with hospital policy and legal requirements.
3. Answers telephone and mail inquiries regarding release of medical information.
4. Ensures compliance with legal requirements for patient inspection and/or copies of medical records.
5. Assists with management of workflow to copy service.
6. Processes subpoenas for records in absence of Clerk IV in coordination with Assistant Manager.
7. Answers telephones, and acts as department receptionist as needed.
8. Greets and assists callers, visitors, physicians, and other staff promptly professionally and courteously.
9. Interacts with callers and visitors in such a manner that provides a positive, caring, professional image.
10. Retrieves records promptly in response to hospital requests.
11. Prioritizes work based on patient care needs.
12. Participates in, or completes all required in-service training or education in a timely manner
13. Meets productivity corporate guidelines
14. Submits weekly productivity reports.

. Performs other related duties as assigned to ensure smooth functioning department, assisting in relief capacity or as regularly assigned duties.
Qualifications: Minimum knowledge, skills and abilities required:
- High school diploma, GED equivalent, or higher level of completed education from an accredited institution required.
- One semester or equivalent experience in use of computers.
- One college level semester of medical terminology required.
- Mental application involving variations in procedure and frequent interruptions. Some exacting concentration required.
- Works with productivity expectations and deadlines to meet departmental goals.
- This position consists of primarily, sitting, with some walking, reaching, light lifting and sufficient manual dexterity to file/retrieve records and operate office machines and computers.
- Good vision is required to read reports and records and operate office machines safely.
Minimum Experience:
1. Minimum 2 years experience in acute hospital Health Information Management or medical experience in other healthcare organization.
2. Ability to learn and utilize computerized patient care management system.
3. Ability to understand and follow verbal and written instructions. Must effectively read, write and verbally communicate in the English language.
