Responsible for providing patient/family care
:
Responsible for providing patient/family care in consultation with Interdisciplinary Team members in accordance with Federal/State Hospice Regulation. Coordinates and develops care plan with IDT: social workers, chaplains and volunteers, CNA's, and other ancillary services. Reviews/evaluates patients in light of Local Medical Review Policies. Educates patient, family and community regarding hospice philosophy and palliative treatments. Provides for appropriate ordering and delivery of medical equipment, pharmaceuticals, and medical supplies. Provides timely and accurate electronic medical documentation. Hours of work are generally M-F 8am to 5pm with a weekend shift rotating schedule of Sat or Sun 8am to 5pm and occasional overtime. Overnight on call coverage is also required

1 - 2 Years Experience
:
Graduate of an accredited school of nursing with current Texas license. Bachelors degree in nursing with home health, hospice, or public health nursing experience preferred. Currently licensed as an RN in the State of Texas. Current certification in basic life support. Excellent administrative skills with ability to supervise staff and coordinate services. Excellent interpersonal and communication skills, both oral and written. Ability to accommodate multi-faith beliefs and backgrounds of patients/families. Ability to function as part of interdisciplinary team. Ability to travel to multi-work locations during workday. Knowledge of applicable State/Federal laws governing hospice licensure. Must have reliable vehicle, valid Texas Drivers License and Liability insurance. PC experience preferred.

Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination. Requires standing and walking for extensive periods of time. Must regularly lift and/or move up to 25 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds. Requires corrected vision and hearing to normal range. Requires working under stressful conditions and working irregular hours. Requires some exposure to communicable diseases or bodily fluids.
